If you would like to see any information that is kept on files about you, please contact the Support Staff. We will only share information on this Support Plan with others where you agree that we can. You have the right to discuss matters with support staff in confidence and the right to be treated fairly, equally and in the knowledge that your cultural beliefs will be respected.
WHY DO YOU NEED A NEEDS ASSESSMENT AND SUPPORT PLAN
It will help you to identify things that you need support with and to plan who is going to do what, how, when. It shows how we agree to measure the progress that has been made on meeting this need and when we will know it has been met. It will also help us to provide you with the most appropriate support to help you maintain your independence and wellbeing. The Housing Benefit Dept require us to record and monitor services we provide to you, and to demonstrate that they are appropriate to your individual needs and wishes. This document will form part of that evidence.
WHAT IF YOUR CIRCUMSTANCES CHANGE?
We will review your support when necessary, however, you are most welcome to discuss them at any time. If you have any major changes in your circumstances we will automatically review your support needs and compile a new support plan with you.
